Upon request, prices are quoted in writing for single shipments or standing orders. Payment
Terms of payment are NET THIRTY (30) days, pending credit approval, payable in U.S. Dollars. Until credit has been established payment will be PREPAY or C.O.D.. A service charge of 1.5% per month will be added for past due balances. Warranties
New Horizons Diagnostics Corporation guarantees the quality of its reagents if used and stored as recommended . Any reagents found to be defective will be replaced free of charge upon return of the product. New Horizons Diagnostics Corporation disclaims any implied warranty of merchantability or fitness for a particular purpose, and in no event shall New Horizons Diagnostics Corporation be liable for consequent damage.

Charges relating to training, are not included in prices unless noted in writing.

Return Policy
A restocking fee of fifteen percent (15%) of the selling price applies to all items for exchange or credit unless an error was made by New Horizons Diagnostics Corporation. Partially used product which has exceeded the expiration date for use cannot be returned.Damaged ProductThe delivering carrier is responsible for damage in shipment. Packaging material should be kept if damage occurs. Immediately contact delivering carrier for inspection of damage and claim filing. Damaged items cannot be returned without authorization.

LUMINESCENCE SERVICE

Type of Service
All work is performed in Columbia, Maryland. In addition to repair or replacement of malfunctioning parts, each instrument is thoroughly checked, cleaned, tested and calibrated as part of the basic service.

Service Rate
$150.00 per hour plus parts (if needed). Minimum charge is two (2) hours.

Loaner Instruments
Loaner instruments are usually shipped within 24-48 hours of request, subject to availability. Loaners are strictly limited to service of customer's unit by New Horizons Diagnostics Corporation. Loaner unit may not be identical to customer‚s unit in all circumstances.

Program
If an instrument requires service or repair, please call New Horizons Diagnostics Corporation to describe problem(s) and schedule service. A purchase order is required before service can be performed, and instruments cannot be accepted without prior authorization by New Horizons Diagnostics Corporation. Upon system arrival, New Horizons Diagnostics Corporation will determine the extent of the repair, and phone/fax the customer with details, as well as an estimated cost. In order to begin work, New Horizons Diagnostics must receive a copy of the above estimate signed by an authorized customer representative.

Shipping Charges
All freight and insurance charges (including loaner units) will be billed to the customer in addition to service charges (warranty and service contract repairs excluded).

Warranty Period
All new instruments are sold by New Horizons Diagnostics with a full one (1) year warranty on parts and labor from the date of initial installation. Please refer to manufacturers warranty for complete details
