To place an order
Issue a purchase order number and identify each item by catalog number and product description. Please specify both a shipping and billing address, as well as any special requirements.

International Shipments
Provide the name, address, telephone number, fax number and, if necessary, contact name for the importing broker.

Quotations
Upon request, prices are quoted in writing for single shipments or standing orders. To order from an accepted price quote, Please reference the quote number to avoid pricing and shipping errors. Credit cannot be extended for an order if the quote is not referenced. New Horizons Diagnostics Corperation reserves the right to adjust the price according to the actual quantities shipped. Quotes are valid for the specified period of time indicated.

Shipping charges
All shipments are FOB shipping point, unless otherwise stated.

Payment
Terms of payment are NET THIRTY (30) days, pending credit approval, payable in U.S. Dolllars. Until credit has been established. Visa, MasterCard, or C.O.D. Warranties
New Horizons Diagnostics Corporation guarantees the quality of its reagents if used and stored as recommended. Any reagents found to be defective will be replaced free of charge upon return of product. New Horizons Diagnostics Corporation disclaims any implied warranty of merchantability or fitness for a particular purpose, and in no event shall New Horizons Diagnostics Corporation be liable for consequent damages.

Training
Charges relating to training are not included in prices unless noted in writing.

Return Policy
All returns require prior authorization by New Horizons Diagnostics Corporation. A restocking fee of fifteen percent (15%) of the selling price applies to all items for exchange or credit unless an error was made by New Horizons Diagnostics Corporation. Partially used product which has exceeded the expiration date for use cannot be returned.

Damaged Product
The delivering carrier is responsible for damage in shipment. Packaging material should be kept if damage occurs. Immediately contact delivering carrier for inspection of damage and claim filing. Damaged items cannot be returned without authorization.
